☐Sample of work (Artists) (Three to five pages of poetry or ten double-spaced pages of prose or script (may combine poetry and prose for maximum of 10 pages), 12-point font minimum. Photographs and photographs of artwork embedded or as separate pages, or links to websites. For video or audio material, include links to example)
☐Sample of work (Academia and Entrepreneurship) (Current CV/resumé in addition to appropriate work sample of 10 pages maximum)
☐Curriculum vitae (To include relevant artistic/academic/business experience: courses, workshops, residencies, publications, projects, or any other relevant activities. Optional for artists but essential for academics/entrepreneurs.)
☐Artist's Statement or work trajectory description (Describe where you are in your life/work trajectory)
☐Statement of need and project description (Explore your need for residency experience, how a solo residency at Holly House will benefit you, why you have chosen to apply to Hypatia-in-the-Woods, what project(s) you will work on, and what outcomes you hope to achieve)
☐References (Names, relationship to you, and email addresses for three people familiar with your work. At least one reference must be from a person qualified to discuss the quality of your work.)
☐Signature on House Rules agreement (Statement that you have read the House Rules and agree to abide by them)
